COMPLETE the “Application for Admission and Rental Assistance” form:
Make sure you include ALL housing and income information, including landlord contact address & numbers, for the past 5 years. Failure to do so will result in rejection of your application.
IF YOU ARE EMPLOYED, you must provide most recent 4-6 consecutive check stubs.
IF you receive Social Security, SSI, or AFDC, provide copies of the latest Award letters.
IF you receive a pension, provide a copy of your most recent statement
IF you are 62 or older, or on disability, provide medical insurance premiums, prescription costs, and any other ongoing unreimbursed medical expenses you have, also known as out-of-pocket.
In addition, HUD provides the owner/agent with information about an applicant's current status as a HUD housing assistance recipient. The owner/agent will use the Enterprise Income Verification System to determine if the applicant or any member of the applicant household is currently receiving HUD assistance. Nothing prohibits a HUD housing assistance recipient from applying to this property. However, the applicant must move out of the current property and/or forfeit any voucher before HUD assistance on this property will begin. Until current HUD housing assistance is properly terminated, resident must pay FULL MARKET RENT on the new unit.
If the applicant or any member of the applicant household fails to fully and accurately disclose rental history, the application may be denied based on the applicant's “misrepresentation” of information.
Return the completed Application to: 5370 Pershing Avenue, St. Louis, MO 63112, and include:
A valid current photo ID,
Social security card(s),
Proof of income, and
Proof of citizenship (U.S. birth certificate), or Proof of eligible non-citizen status
Please note that if any section of the application is left blank, your application will be rejected.
If a section of the application does not apply to you, you must write ‘N/A’ in the section.
